Are you asking about Medics/EMT's that work directly for the hospital or that work for a separate EMS agency that happen to be based at a hospital?
Inquiring minds want to know.

I work on an ambulance that is hospital based. My service was hospital bases way before my time. Back when the maintenance guys had to become EMT's and ran calls, it was only a BLS service then. We are the red-headed step children of the hospital. My surpervisors supervisor is a RN that has no clue about EMS, makes things interesting to say the least. We also as a department have to support those departments that don't turn revenue.

I work for a hospital based ALS intercept service. I feel that our department is generally supported by the administration except when it comes to our role inside the hospital, but that is probably a whole different topic. Anyway, I think that the administration supports the departments that they get things back from. For EMS, a hospital based system can be a good public relations tool for the hospital. Take the initiative and contact the local newspaper or whoever and get your names and faces out there. I am infuriated by my coworkers who complain about low pay and lack of respect for the profession and then turn into cockroaches running out of the light whenever a camera comes by. If you want support, higher pay and respect you have to take it. You have to step outside of the comfort of the lounge, find yourself sweating and panic stricken staring into a camera. Otherwise, people are not going to just magically learn what we do. People don't know and it's up to you and me to educate them. So, that's my two cents on that.

There are some GREAT hospital-operated services in the United States. Many of the NYC hospitals provide ambulances to the 911 system. Jersey City and Newark (University Hospital) are outstanding organizations - UMDNJ operates in addition to ambulances the first hospital-operated heavy EMS rescue in the U.S. (since the 1980s). Wishard Memorial EMS in Indianapolis is another one.

On the other end of the spectrum there is a truly outstanding rural hospital EMS service in Winnemucca, Nevada, that has received lots of positive press lately.
